ATutor
ATutor is a comprehensive web based tutorial system, designed for use in all types of classes. It has support for importing tutorials from other tutorial systems that meet certain standards. It also includes support for a large number of navigation styles, accesibility options, group works, and even a skinnable web interface.

Bad Features:

tests are not part of the tutorial, they are seperate. They are not used as a "check what you've learned" thing at the end of a tutorial, they are instead a graded test or quiz for use in class evaluation, like those provided by the test tool.

assist boxes in the editor on all newly created courses get annoying after creating a few courses
Missing Features:

remote desktop functionality

Overall, this system provides many of the features requested, but it does not quite have everything.
